Description
Shown from a posterior perspective, the lateral portion of semispinalis capitis is traced as a thick column of muscle fibers running superomedially from the upper thoracic and lower cervical spinous and transverse processes toward the occipital bone, adjacent to the midline nuchal region. Superiorly, its insertion lies between the superior and inferior nuchal lines, deep to the more superficial splenius capitis and trapezius in anatomic layering. Medially it approaches the spinous processes and the nuchal ligament, while laterally it borders the longissimus capitis and sits medial to the scapular contour visible at the upper back. Bone landmarks include the posterior elements of the cervical vertebrae, the cervicothoracic junction, and the occipital squama. Semispinalis capitis is a prime extensor of the head and a strong contralateral rotator, and this posterior view makes its fiber direction easy to teach against the underlying cervical spine. Clinically, the muscle is a frequent generator of cervicogenic headache and occipital pain, with trigger points and local hypertonicity often referring pain toward the vertex; its deep surface also forms part of the corridor where the greater occipital nerve emerges and can be irritated in occipital neuralgia. Surgeons working through a midline posterior cervical approach rely on this anatomy when elevating paraspinal muscles from the spinous processes to expose the laminae for decompression or fusion. A key landmark. Use this asset for teaching posterior neck muscular layers in gross anatomy, kinesiology, and spine modules, or for illustrating cervical extensor strain and headache mechanisms in neurology, pain medicine, and physical therapy publications.
