Description
Laterally, the semispinalis capitis appears as a deep posterior neck muscle running superiorly from the upper thoracic and lower cervical region toward the occipital bone, positioned medial to the more superficial splenius capitis and posterior to the cervical vertebrae. Its fibers course obliquely upward and medially to insert between the superior and inferior nuchal lines of the os occipitale, closely related to the midline nuchal ligament and the spinous processes of C2 through C7. Adjacent bony landmarks include the occiput, cervical vertebral arches, and the superior angle and medial border of the scapula providing inferior context. Deep layers such as the semispinalis cervicis and the cervical multifidus are implied beneath it. A deep extensor. This lateral emphasis matters because semispinalis capitis is a frequent pain generator in cervicogenic headache and neck extension strain, and its relationship to the greater occipital nerve helps explain occipital neuralgia patterns when deep paraspinal tone is increased. Palpation and trigger point referral often localize near the superior nuchal line, while posterior cervical approaches, including C2 instrumentation corridors, demand a clear mental map of the deep extensors you must split and later repair to reduce postoperative weakness.
