Description
Prominent in the posterior upper back, the semispinalis thoracis is highlighted deep to trapezius and the posterior shoulder girdle musculature, tracking superomedially from the transverse processes of upper thoracic vertebrae to the spinous processes several levels above. Its slips span multiple segments, crossing the midline to insert on thoracic and lower cervical spinous processes, immediately medial to the longissimus thoracis and deep to the rhomboid major near the medial border of the scapula. Surrounding landmarks include the scapulae laterally, proximal humeri at the glenohumeral region, and the thoracic spinous processes forming the midline ridge. Deep, multisegmental fibers dominate the field. Semispinalis thoracis sits within the transversospinalis group and drives ipsilateral extension with contralateral rotation, a pattern clinicians test when differentiating segmental paraspinal pain from scapulothoracic sources. Because its attachments bridge several vertebrae, the muscle also helps explain why thoracic facet (zygapophysial) joint irritation or costovertebral dysfunction can refer pain paraspinally and toward the medial scapular border, sometimes mimicking rhomboid strain. Palpation and needle trajectory matter. This view supports teaching safe posterior approaches by keeping orientation to the spinous processes and medial scapular border while appreciating the depth of the transversospinalis layer.
