Description

Arising from the inferior mesenteric artery, the sigmoid arteries course inferiorly and leftward to enter the sigmoid mesocolon, where they form characteristic arterial arcades before giving off straight vasa recta to the S shaped sigmoid colon. From an anterior perspective, the sigmoid colon sits in the left lower quadrant, anterior to the sacrum and medial to the left pelvic brim, with the descending colon continuing superiorly along the left flank. The inferior mesenteric vein and accompanying venous tributaries run in parallel but typically slightly posterior to the arterial branches, while autonomic nerve fibers track alongside the mesenteric vessels toward the colonic wall. Arcades dominate the mesenteric field. Clinical interest centers on the variable number and caliber of these sigmoid branches and their relationship to the marginal artery of Drummond at the colonic border, a pattern that influences perfusion after inferior mesenteric artery ligation during left sided colectomy or sigmoidectomy.
