Arising from the distal descending colon and sigmoid colon, the sigmoid veins form a bluish venous arcade along the mesenteric border, then converge medially within the sigmoid mesocolon to reach the inferior mesenteric vein. From an anterior perspective, the sigmoid loop occupies the left lower quadrant, inferior to the transverse colon and lateral to the midline small-bowel mesentery, with haustrated colonic wall detail guiding the segmental anatomy. Superiorly, portions of the stomach and liver sit above the colonic frame, while the iliac bones bracket the pelvic inlet and anchor the viewer’s orientation. Sigmoid venous anatomy matters when you are tracking the portal venous drainage of the hindgut and anticipating collateral pathways. These vessels feed the inferior mesenteric vein and then the splenic vein, so they sit squarely in the hemodynamic circuit affected by portal hypertension and by left-sided portal obstruction (for example, splenic vein thrombosis), where venous engorgement can complicate dissection.
