Description

Prominent on the anterior surface, the anterior cardiac veins course over the right ventricle and converge toward the right atrium, running in shallow epicardial grooves alongside small arterial branches on the sternocostal aspect of the heart. Superiorly, the ascending aorta and pulmonary trunk rise from the base, while the right atrial auricle overlaps the anterosuperior right ventricle. Medially, the anterior interventricular sulcus provides a clear landmark for separating right and left ventricular territories. Anterior cardiac veins matter because they are a frequent exception to the usual “all roads lead to the coronary sinus” teaching point. Many drain directly into the right atrium through small ostia, bypassing the coronary sinus, so this view supports accurate explanation of venous return from the right ventricular free wall and why coronary sinus catheterization does not sample all cardiac venous blood.
