Description
Running in blue over a white anterior heart, the great cardiac vein tracks up the anterior interventricular sulcus alongside the left anterior descending artery, then turns leftward in the atrioventricular (coronary) sulcus toward the left atrium. Diagonal tributaries from the anterior wall of the left ventricle and small cardiac venous branches from the right ventricle converge toward this main channel, with the right atrium and right ventricle positioned anteriorly and the left ventricle forming the left border and apex. Superiorly, the ascending aorta and pulmonary trunk provide midline landmarks, while the auricular margins frame the base. Clean vessel mapping. Anterior coronary venous anatomy matters when you need to predict where blood and devices travel after the myocardium has done its work. The great cardiac vein and its continuation into the coronary sinus define the most common route for left ventricular lead delivery in cardiac resynchronization therapy, and this simplified view helps you teach why a difficult venous angle at the atrioventricular groove or a small-caliber tributary can limit lead placement. It also supports surgical orientation during anterior approaches where epicardial veins can be injured or mistaken for arterial branches during coronary artery bypass grafting.
