Description
Arising from the hila of the right and left lungs, the superior and inferior pulmonary veins course medially to enter the left atrium on its posterior aspect, yet their ostia are clearly appreciated from an anterior orientation when the heart is presented with the great vessels. Paired veins on each side flank the pulmonary arterial branches, with the right pulmonary veins running posterior to the superior vena cava and right atrium and the left pulmonary veins passing anterior to the descending thoracic aorta as they approach the atrial wall. Color separation between arterial and venous channels makes the pulmonary venous return easy to follow in relation to the aortic root and pulmonary trunk. Clean anatomy. Clinically, pulmonary vein anatomy matters most when you need precise left atrial landmarks. Catheter ablation for atrial fibrillation targets the myocardial sleeves extending into the pulmonary veins, and variant venous drainage patterns (common left pulmonary venous trunk or an extra right middle pulmonary vein) change both mapping strategy and the risk of post-procedural pulmonary vein stenosis. This anterior framing also helps orient learners to why pulmonary veins carry oxygenated blood despite being veins, a recurring point of confusion in introductory cardiopulmonary teaching.
