Description
Arising from the superior poles of the right and left thyroid lobes, the superior thyroid veins course superiorly and slightly laterally along the anterior surface of the gland, draining venous blood from the upper thyroid parenchyma. The paired lobes sit on either side of the upper trachea and laryngeal framework, joined across the midline by the isthmus, which lies anterior to the second and third tracheal rings. An anterior perspective also implies close relationship to the infrahyoid (strap) muscle plane that normally overlies the gland, and to the superior thyroid artery and external branch of the superior laryngeal nerve in the superior pedicle region. Small venules and a superficial venous plexus may be suggested over the capsule. Dense vascular territory. For thyroidectomy and lobectomy planning, venous outflow matters as much as arterial control because the superior thyroid veins tether the superior pole and can obscure the superior pedicle where the external branch of the superior laryngeal nerve is at risk. Early identification and ligation of these veins helps reduce troublesome oozing during capsular dissection, and it clarifies the surgical window around the cricothyroid muscle and superior thyroid vessels. The same anatomy explains why upper pole manipulation can precipitate voice fatigue in patients when the nerve is stretched or thermally injured.
