Description
Frontal anatomy frames the right lung with three lobes and the left lung with two, flanking a midline trachea that bifurcates at the carina into the right and left main bronchi. From each hilum, pulmonary veins course medially and slightly posterior toward the left atrium, collecting oxygenated blood from progressively smaller venules within the lung parenchyma. Superior and inferior pulmonary veins are implied on each side, with the right-sided veins typically taking a shorter, more direct path across the mediastinum than the left. Blue bronchial branching contrasts with the venous return. Clear landmarks. Pulmonary veins matter because they define the only venous system that normally carries oxygenated blood, and their ostia at the left atrium are frequent targets in electrophysiology. Catheter-based pulmonary vein isolation for atrial fibrillation relies on an accurate mental map of right versus left, superior versus inferior venous entry, and on common anatomic variants such as a left common pulmonary venous trunk. Surgical and imaging correlation is equally practical: CT angiography and transesophageal echocardiography often reference the anterior relationship of the pulmonary veins to the bronchi at the hilum and their posterior course into the atrium when assessing stenosis, post-ablation narrowing, or anomalous pulmonary venous return.
