Presented from an inferior (diaphragmatic) perspective, the male heart is oriented to emphasize the coronary venous network coursing over the posterior surfaces of the ventricles. The coronary sinus runs in the posterior atrioventricular (coronary) sulcus, receiving the great cardiac vein as it curves from the left margin and the middle cardiac vein ascending in the posterior interventricular sulcus toward the crux cordis. Smaller tributaries, including posterior veins of the left ventricle and the small cardiac vein along the right margin, track superiorly toward the right atrium, where the coronary sinus ostium lies just medial to the tricuspid valve annulus. An inferior view matters because it is where venous drainage patterns are easiest to teach and where anatomic variability carries procedural consequences. Cannulation of the coronary sinus for cardiac resynchronization therapy, retrograde cardioplegia, or electrophysiology mapping depends on recognizing the coronary sinus course and its relationship to the atrioventricular groove and the posterior interventricular region. Misidentifying a prominent middle cardiac vein or a large posterior left ventricular vein can complicate lead placement or alter the distribution of retrograde perfusate. Short and practical.
