Description
Posterior cardiac anatomy is rendered in semi-transparent white with the coronary venous system picked out in blue against the left and right ventricles. The coronary sinus courses in the posterior atrioventricular (coronary) sulcus, running transversely and slightly left to right before opening into the right atrium near the inferior vena cava. Tributaries that logically stand out in this projection include the great cardiac vein turning onto the posterior surface, the middle cardiac vein ascending in the posterior interventricular sulcus, and the small cardiac vein tracking along the right margin toward the coronary sinus. Posterior emphasis matters because the coronary sinus and its ostium are the working corridor for multiple electrophysiology and structural heart procedures. Cannulation of the coronary sinus for left ventricular lead placement in cardiac resynchronization therapy depends on recognizing the ostium’s position inferior and posterior in the right atrium and anticipating the angles of entry from the great and middle cardiac veins. The same anatomy guides retrograde cardioplegia delivery during cardiac surgery and helps explain why coronary sinus thrombosis or injury can compromise venous drainage from the left ventricular myocardium.
