Description
Centered in the cranial base, the sphenoid is presented in superior aspect, with the body flanked laterally by the right and left greater wings and capped anteriorly by the lesser wings. Between the lesser wings lies the sulcus chiasmatis (chiasmatic groove), which runs transversely just anterior to the sella turcica, while the tuberculum sellae rises at the anterior margin of the hypophyseal fossa and the dorsum sellae defines its posterior wall. Along the medial ends of the lesser wings, the optic canals open toward the orbital apices, and the superior orbital fissures separate the lesser wing superiorly from the greater wing inferiorly. A critical midline landmark. Neurosurgeons and neuroradiologists rely on this top-down anatomy to orient the pituitary region: the sella turcica maps directly to the position of the pituitary gland, and the sulcus chiasmatis corresponds to the course of the optic chiasm, the structure classically compressed by a pituitary macroadenoma with resultant bitemporal hemianopia. The relationship between the anterior clinoid processes (at the medial lesser wings), optic canals, and superior orbital fissures also frames common corridors for anterior clinoidectomy and approaches to parasellar aneurysms. Useful for teaching skull base osteology in medical and dental gross anatomy, for labeling plates in neurosurgery or endocrinology texts on pituitary disease, and for briefing learners on endoscopic endonasal transsphenoidal surgery landmarks when paired with sinus and sellar views.
