Centered within the vertebral canal, the spinal cord is presented in transverse section with butterfly-shaped gray matter surrounded by circumferential white matter. Dorsal (posterior) horns project toward the posterior median sulcus, while broader ventral (anterior) horns extend toward the anterior median fissure, and the central canal sits at the gray commissure in the midline. Encircling the cord, cerebrospinal fluid (CSF) fills the subarachnoid space, bounded peripherally by the meningeal layers that separate neural tissue from the bony vertebra. Clear compartmental anatomy. Clinical teaching often hinges on this exact arrangement because lesion patterns map cleanly onto horns and funiculi: anterior horn involvement produces lower motor neuron weakness, while posterior column disruption affects proprioception and vibration, and anterolateral tract compromise shifts pain and temperature loss. The CSF ring matters too, since it illustrates the hydrostatic buffering that is lost when the subarachnoid space is obliterated by tumor, epidural abscess, or severe spinal stenosis.
