Description
Centered on the superior mediastinum and cardiac base, the subaortic (aortopulmonary window) lymph nodes are clustered inferior to the aortic arch and lateral to the ligamentum arteriosum, lying between the arch and the left pulmonary artery. An anterior cardiac view also brings the ascending aorta and proximal aortic arch into relief, with the pulmonary trunk bifurcating into right and left pulmonary arteries just inferior to the arch. Coronary arteries and veins course over the epicardial surface, while green lymphatic vessels accompany the great vessels and drain toward the tracheobronchial chain. Small structures, tight space. These nodes matter because they sit on a common pathway for lymphatic drainage from the left lung and mediastinum, so they frequently appear in lung cancer staging and in granulomatous disease such as tuberculosis or sarcoidosis. Their position in the aortopulmonary window also explains why left recurrent laryngeal nerve palsy can accompany nodal enlargement, as the nerve hooks under the aortic arch near the ligamentum arteriosum. This anterior perspective clarifies why surgeons and interventionalists reference the aortic arch, pulmonary artery, and ductus/ligamentum complex when describing access to the region.
