Sulcus Precentralis, Anterior View

Description

Facing the cerebrum anteriorly, the paired cerebral hemispheres frame the interhemispheric fissure while the frontal poles sit superior to the orbits and anterior to the brainstem. The sulcus precentralis (precentral sulcus) is identified on each hemisphere as a longitudinal groove in the frontal lobe that lies immediately anterior to the sulcus centralis, creating the anterior boundary of the gyrus precentralis. Medially it approaches the superior margin near the lobulus paracentralis, and laterally it trends toward the inferior frontal gyrus where surface patterns become more variable. Gyri and sulci are rendered with enough relief to appreciate their continuity across the convexity. Locating the precentral sulcus matters because it is the most reliable superficial landmark for bracketing the primary motor cortex on the gyrus precentralis when planning or teaching functional anatomy. In tumor resection, epilepsy surgery, and perirolandic arteriovenous malformation work, confusing the sulcus precentralis with the sulcus centralis can shift a cortical target by a full gyrus, with predictable postoperative deficits in contralateral face, hand, or leg strength depending on the homuncular level. The anterior viewpoint also reinforces how variable secondary sulci can be, and why surgeons correlate these grooves with intraoperative mapping, neuronavigation, and preoperative fMRI rather than trusting symmetry alone. Neuroanatomy and neurosurgical trainees will find this orientation useful for exam preparation on frontal lobe surface anatomy, and for explaining “perirolandic” terminology in lectures that bridge gross anatomy with motor localization.
