Description
Beneath the reflected posterior skin of the male head and upper neck, the superior obliquus capitis (obliquus capitis superior) runs superomedially from the transverse process of the atlas (C1) to the lateral inferior nuchal line of the occipital bone. Superficial to it, the trapezius and deeper posterior cervical layers such as splenius capitis and semispinalis capitis frame the suboccipital region, while the occipital squama and nuchal lines provide fixed bony landmarks superiorly. Blue nerve elements course in the same field, consistent with the dorsal rami in this compartment, including the suboccipital nerve (C1) in close proximity to the muscle and the greater occipital nerve (C2) ascending more medially toward the posterior scalp. Orientation matters here because obliquus capitis superior forms the superolateral boundary of the suboccipital triangle, a small but clinically busy corridor where the vertebral artery turns medially across the posterior arch of C1 before entering the foramen magnum. Small errors in identifying the atlas, inferior nuchal line, and adjacent muscles can translate into poor surface-to-depth mapping during posterior C1–C2 approaches, occipital-cervical fusion exposure, or targeted injections for cervicogenic headache. Palpation-based teaching also benefits from this view, since the muscle sits deep but defines the palpable contour just inferior to the occiput when the head is extended and rotated. A tight space.
