Description
Centered in the upper posterior neck, the obliquus capitis superior runs from the transverse process of the atlas (C1) superomedially to the occipital bone between the inferior nuchal line and the foramen magnum. Superficial to it, the skin and subcutaneous tissue drape over the posterior cervical contour, while deeper and slightly medial you would expect the rectus capitis posterior major and minor forming the layered suboccipital musculature. Lateral and inferior to the muscle belly, cervical vessels course vertically toward the scalp, consistent with the pathway of the occipital arterial and venous network in the nuchal region. Deep anatomy matters here. This posterior exposure is the teaching workhorse for the suboccipital region because the obliquus capitis superior defines the superolateral boundary of the suboccipital triangle, where the vertebral artery loops in the groove on the posterior arch of C1 before entering the cranial cavity. That relationship is not academic: posterior C1 instrumentation, deep trigger point injection, or a surgical approach to the craniovertebral junction risks vascular injury if the atlas level is misread by a few millimeters. Neural pain patterns track the same space, with irritation of the suboccipital nerve (dorsal ramus of C1) and proximity to the greater occipital nerve (C2) often discussed in cervicogenic headache and occipital neuralgia. Use this illustration in gross anatomy labs when teaching the layered posterior neck (splenius capitis to semispinalis capitis to suboccipital muscles), and in neuroanatomy or spine modules when correlating C1 rotation with atlanto-occipital and atlanto-axial biomechanics.
