Description
Beneath the posterior scalp and upper cervical fascia, the obliquus capitis superior (superior obliquus capitis) runs superolaterally from the transverse process of the atlas (C1) to the occipital bone between the inferior nuchal line and the foramen magnum. Deep to the semispinalis capitis, its short fibers sit in the suboccipital region just superior to the posterior arch of C1 and lateral to the midline attachment area of rectus capitis posterior minor. The atlas and axis (C2) frame the atlantoaxial segment inferiorly, while the occiput forms the superior boundary at the craniovertebral junction. A small but decisive muscle. Functionally, this orientation explains why the muscle is a consistent landmark in teaching fine head movements, it contributes to ipsilateral lateral flexion and assists extension at the atlanto-occipital joint, while the axis provides the adjacent rotary mechanics of the atlantoaxial joint. Posterior approaches to the craniovertebral junction, including C1 lateral mass fixation or exposure around the posterior arch, pass through the same layered interval where the suboccipital muscles define safe planes. The suboccipital nerve (dorsal ramus of C1) courses in this compartment, and irritation in this region is a recognized contributor to cervicogenic headache patterns and occipital pain referral.
