Description
Posterior neck anatomy is rendered with the occipital bone and cranial base superiorly and the upper cervical spine inferiorly, emphasizing the suboccipital compartment. The superior obliquus capitis lies deep to the superficial nuchal soft tissues, running superomedially from the transverse process of the atlas (C1) to the lateral part of the inferior nuchal line of the occipital bone, lateral to the rectus capitis posterior major and minor. C1 and C2 define the atlantoaxial region immediately deep to these short muscles, with the posterior arch of the atlas positioned just inferior to the occiput and the axis (C2) lying inferior and slightly anterior by comparison. Deep and compact. This posterior exposure matters because the superior obliquus capitis is a key boundary of the suboccipital triangle, where the vertebral artery curves medially in the groove on the posterior arch of C1 before entering the foramen magnum, and where the suboccipital nerve (dorsal ramus of C1) emerges to supply these muscles. Hypertrophy, spasm, or iatrogenic trauma in this layer is a common discussion point in cervicogenic headache mechanisms and in posterior C1 lateral mass instrumentation, where staying oriented to C1 transverse process and the inferior nuchal line helps avoid vertebral artery injury. The atlanto-occipital and atlantoaxial joints are also taught cleanly from this relationship: extension at the occiput on C1, rotation concentrated at C1 on C2.
