Talus anatomy is centered here on the sulcus for the tendon of flexor hallucis longus along the inferior aspect of the posterior process. In this inferior view, the groove sits between the medial and lateral tubercles of the posterior process, forming a deep channel that guides the flexor hallucis longus tendon as it courses from posterior to anterior toward the plantar foot. Anterior to the groove, the plantar surface transitions into the posterior calcaneal articular facet for the subtalar joint, while the sulcus tali and adjacent facets provide bony context for the tarsal canal region. Bone contours dominate the frame. Flexor hallucis longus (FHL) irritation is a common pain generator in dancers and athletes, and the talar groove is a frequent site of stenosing tenosynovitis where repetitive plantarflexion loads the tendon against the posterior process. Variants such as an enlarged posterolateral tubercle (Stieda process) or an accessory bone (os trigonum) can narrow this channel and contribute to posterior ankle impingement, while fractures of the posterior process (including Cedell fracture of the posteromedial tubercle and Shepherd fracture posterolaterally) place the FHL tendon at risk for entrapment or scarring. The fixed inferior perspective makes the relationship between the groove, the tubercles, and the subtalar articular surface easy to teach when correlating radiographs, CT, or hindfoot arthroscopy landmarks.
