Description

Arising from the abdominal aorta inferior to the renal arteries, the testicular (gonadal) artery courses obliquely inferiorly on the posterior abdominal wall toward the deep inguinal ring. From a posterior perspective it lies retroperitoneally, passing anterior to psoas major and crossing the ureter, a relationship often remembered as water under the bridge, with the ureter typically posterior to the gonadal vessels. Nearby, the kidneys sit superior and lateral, while lumbar vertebral bodies and adjacent musculature frame the course of the vessel; branches of the lumbar plexus, including the genitofemoral nerve on the anterior surface of psoas, run in close proximity. Small but distinct. This angle matters when you need to teach or plan around the retroperitoneal corridor where vascular, ureteric, and neural structures crowd together, and where a misplaced clip or cautery bite can create ischemia or postoperative scrotal pain. During varicocelectomy, inguinal or laparoscopic hernia repair, and retroperitoneal lymph node dissection for testicular cancer, identifying the gonadal artery and its crossing with the ureter helps prevent testicular atrophy from arterial injury and ureteral devascularization. The posterior view also clarifies how the gonadal vessels relate to the renal hilum and to lumbar plexus elements that can be irritated by retraction.
