Description
Arising from the posterior aspect of the external carotid artery, the occipital artery ascends deep to the posterior belly of the digastric and stylohyoid, then runs posteriorly along the mastoid process toward the superior nuchal line. From a posterior head and neck perspective, its tortuous scalp course is seen climbing superior to the occiput, with occipital branches fanning over the posterior scalp superficial to the suboccipital musculature and trapezius insertion. Flanking landmarks such as the auricles and mastoid region orient the vessel laterally, while midline structures over the external occipital protuberance and nuchal ligament sit medial to the artery’s terminal branches. Cutaneous nerve trajectories, consistent with the greater occipital nerve and adjacent posterior cervical nerves, track in close proximity to the vascular arborization. Posterior scalp vascular anatomy matters when you are planning incisions, flaps, or hemostasis in the occipital region, because the occipital artery can be a persistent source of bleeding after trauma or during posterior scalp surgery. Its relationship to the greater occipital nerve is also clinically relevant in occipital neuralgia and during occipital nerve blocks, where the artery serves as a palpable landmark near the superior nuchal line. Ligation or injury near the mastoid and nuchal attachments can compromise perfusion to posterior scalp flaps. Use this posterior view for teaching head and neck vascular territories in gross anatomy, for surgical atlases covering posterior scalp laceration repair and flap design, or for pain medicine materials illustrating landmark-guided greater occipital nerve blockade.
