Description
Posterior anatomy of the male upper trunk is rendered with the major arterial and venous conduits tracking along the thoracic vertebral column. Running longitudinally just anterior to the vertebral bodies, the descending thoracic aorta gives off paired posterior intercostal arteries that course laterally within the intercostal spaces, while accompanying intercostal veins converge medially toward the azygos system on the right and the hemiazygos and accessory hemiazygos veins on the left. Superiorly, proximal great vessels related to the heart and mediastinum are implied, and distally the thoracic vessels narrow and segment into arterioles and capillary beds before draining back into larger-caliber veins. Posterior vascular views matter when you need to teach or plan around the paravertebral and intercostal corridors. Think thoracotomy and posterior rib approaches, where the intercostal neurovascular bundle sits in the costal groove and uncontrolled bleeding from a posterior intercostal artery can be brisk. The azygos and hemiazygos pathways also frame clinically common detours in superior vena cava obstruction and explain collateral venous return patterns that are easiest to conceptualize from behind. Bleeding risks are not symmetric. Common use cases include gross anatomy and physiology instruction on systemic circulation, surgical education modules on posterior thoracic approaches, and medical publishing figures on intercostal vessel anatomy, azygos system variants, and collateral venous drainage in mediastinal disease.
