Description
Posteriorly oriented tracheal rings form a stacked series of C shaped hyaline cartilages, open along the posterior midline where the membranous wall completes the airway. Spanning the gap between the free ends of each ring, the trachealis muscle and fibroelastic tissue sit immediately anterior to the esophagus, creating the shared boundary of the respiratory tract and upper gastrointestinal tract. Inferiorly, the trachea continues toward the carina and main bronchi, while laterally the airway relates to the lobes of the thyroid gland in the lower neck and to the mediastinal pleura in the upper thorax. This posterior perspective matters because it highlights the noncartilaginous segment that permits dynamic changes in tracheal caliber during cough and forced expiration, and it clarifies why a distended esophagus can indent the airway. It is also the surface encountered conceptually during posterior tracheal wall injury and tracheoesophageal fistula, and it frames endoscopic and surgical discussions of tracheal stenosis, posterior membranous laceration after intubation, and repair techniques that must respect the proximity of the esophagus.
