Description
Shown from a lateral perspective, the left posterior mandible is rendered semi-transparent to expose the third mandibular molar (wisdom tooth) within the alveolar process, with adjacent second molar crowns anterior to it and the mandibular body continuing inferiorly. The highlighted tooth reveals enamel overlying dentin and a central pulp chamber that narrows into pulp canals within the roots. Root morphology is emphasized, including common distal inclination and curvature, with the apices positioned inferior and posterior relative to the cervical line and neighboring molar roots. Root form and angulation of the mandibular third molar are where routine becomes unpredictable. Curved or divergent roots, apical dilaceration, and a narrowed pulp space complicate exodontia, increase the chance of root fracture, and influence decisions around sectioning the crown or creating a distal trough during surgical extraction. The lateral view also supports teaching around iatrogenic risk: deep posterior roots sit near the course of the inferior alveolar canal in the mandibular canal, and unfavorable orientation can correlate with postoperative neurosensory symptoms when extraction is difficult.
