Description
Viewed from a superior, occlusal perspective, the second mandibular molar sits in the posterior dental arch just distal to the first molar and mesial to the retromolar region, with its crown broadest buccolingually. Four principal cusps are visible (mesiobuccal, distobuccal, mesiolingual, distolingual), separated by a central groove that forms the classic cruciform pattern, and the marginal ridges frame the mesial and distal borders. Enamel outlines the occlusal table, while dentin and the pulp chamber lie deep to the cusp tips and fissures. If the surrounding arch is included, the opposing maxillary teeth and the curvature of the maxillary alveolar process may appear as contextual anatomy, even though the highlighted subject is mandibular. Occlusal morphology of the mandibular second molar is a workhorse for teaching dental anatomy because it anchors laterality, cusp identification, and groove patterns used in charting and in restorative design. That cross-shaped fissure is also a predictable site for plaque retention and pit-and-fissure caries, with lesion progression tracking from enamel into dentin and, if untreated, toward the pulp horn beneath the involved cusp.
