Description
Centered within the distal carpal row, the trapezoid (os trapezoideum) is presented in anterior (palmar) view and highlighted against the surrounding wrist skeleton. Medially it articulates with the capitate, laterally with the trapezium, while its distal surface meets the base of the second metacarpal and its proximal surface faces the scaphoid. Adjacent metacarpals, phalanges, and the remaining carpal bones frame the trapezoid in anatomical position. Its wedge-like profile reads clearly. Palmar viewing matters because it aligns with how clinicians mentally map the carpometacarpal region during examination and operative planning, where small differences in carpal geometry influence stability of the index ray. The trapezoid is a keystone for the second carpometacarpal joint, and its strong ligamentous constraints help explain why isolated trapezoid fractures are uncommon but can be missed on standard radiographs, sometimes requiring CT when focal dorsal wrist pain persists after axial loading through the index metacarpal.
