Description
Oriented in anatomical position from an anterior perspective, the radius lies lateral and the ulna medial, extending proximally toward the elbow and distally toward the wrist. A transverse fracture line crosses the proximal forearm, positioned just inferior to the elbow region, with the fracture margins emphasized to distinguish the proximal and distal fragments. Proximally, the ulna’s coronoid process would sit anterior to the trochlear notch, while the radial head and neck align lateral to it at the level of the radiocapitellar joint. Distal landmarks such as the ulnar head and radial styloid process are implied along the same longitudinal axis. Clean break. Proximal forearm fractures in this zone matter because they sit at the crossroads of elbow stability and forearm rotation. A coronoid fracture can compromise the anterior buttress of the ulnohumeral joint, and when paired with injury around the radial head it raises immediate concern for the elbow “terrible triad” pattern (radial head fracture, coronoid fracture, and lateral collateral ligament complex disruption), a scenario that often presents after a fall on an outstretched hand. The anterior viewpoint clarifies how a seemingly simple transverse line can intersect structures that guide pronation and supination and that are scrutinized during open reduction and internal fixation planning.
