Description
Oriented in posterior anatomical position, the ulna lies medial and the radius lateral, with the proximal ends forming the posterior contour of the elbow at the olecranon and the radial head just lateral to it. A fracture line is emphasized near the proximal forearm, and the olecranon fragment is shown displaced, separating from the trochlear notch that would normally cup the humeral trochlea. Red accenting tracks the break margins to guide the eye along the disrupted cortical continuity. Alignment is the story here. Posterior visualization of a proximal ulna fracture with associated involvement of the radial head cues the classic injury patterns that drive management, including Monteggia-type fracture-dislocation where ulnar malalignment can leave the radial head incongruent with the capitulum and unstable on pronation-supination. That relationship matters in the operating room: restoring ulnar length and posterior angulation is often the step that reduces the radiocapitellar joint, while persistent radial head displacement should prompt a search for annular ligament interposition or associated coronoid injury.
