Description
Presented in lateral profile, the paired forearm bones are arranged in anatomical position with the radius lateral (thumb side) and slightly bowed, and the ulna medial with a straighter shaft and a prominent proximal olecranon forming the posterior point of the elbow. Proximally, the radial head sits just distal to the capitulum region, while the ulna’s trochlear notch region is implied by the contour beneath the olecranon. Distally, the radius broadens toward its metaphysis for the wrist joint while the ulna tapers toward the ulnar head and styloid region. Both diaphyses show mid-shaft fracture markings, aligned transverse-to-oblique across the cortical surface. Mid-diaphyseal fractures of both radius and ulna are the classic both-bone forearm fracture, a pattern that disrupts the ring-like relationship between the proximal and distal radioulnar joints and can abolish pronation-supination if length, radial bow, or rotational alignment is not restored. Malreduction risks loss of forearm rotation and can predispose to radioulnar synostosis, especially after high-energy trauma. In adults, this injury often requires plate-and-screw fixation rather than casting, and a clean lateral view helps communicate angulation and displacement at a glance. No room for guesswork.
