Description
Medial elbow anatomy is organized around the ulnar (medial) collateral ligament complex, with the anterior band spanning from the anteroinferior medial epicondyle of the humerus to the sublime tubercle on the medial margin of the coronoid process of the ulna. From a medial perspective, the ligament sits deep to the common flexor-pronator origin, while the ulnar nerve tracks posterior to the medial epicondyle within the cubital tunnel and then courses distally alongside the flexor carpi ulnaris. Proximally, the medial epicondyle forms the key bony landmark, and distally the ulnohumeral articulation and coronoid region anchor the ligament’s ulnar attachment. Small but telling relationships matter here, anterior band fibers lie anterior to the axis of elbow flexion and tighten with valgus load. Clinically, this is the restraint most tested in throwing athletes, where repetitive valgus stress can drive attenuation or rupture of the anterior band and produce posteromedial olecranon impingement. Surgeons reconstructing the UCL often target the sublime tubercle and the medial epicondyle (docking or figure-of-eight techniques), and this medial view clarifies why misplacement risks residual valgus laxity or iatrogenic injury to the ulnar nerve. Stress ultrasound and MRI both aim to interrogate the same fiber direction and attachment sites you see here. It is a common failure point.
