Description
Lateral skin and superficial fascia form the outer envelope, while the vastus intermedius is picked out beneath the more superficial quadriceps, lying deep to rectus femoris and largely hidden by vastus lateralis from this side. Its muscle belly hugs the anterior and lateral surfaces of the femoral shaft, tapering distally toward the quadriceps tendon that courses to the superior pole of the patella and continues as the patellar ligament to the tibial tuberosity. Anteriorly it occupies the central compartment between the lateral and medial vasti, with its fibers running inferiorly toward the knee in a broad, pennate arrangement. Deep. Because the vastus intermedius is the quadriceps layer that contacts the femur directly, this view helps clarify why anterior thigh contusions and femoral shaft fractures can produce painful inhibition of knee extension even when rectus femoris appears intact on surface inspection. Orthopedic exposure for intramedullary nailing, distal femoral plating, or anterior approaches around the suprapatellar pouch must account for the deep quadriceps plane and its relationship to the femoral periosteum and joint capsule, and sports medicine assessments of quadriceps tendon pain often require you to think past the superficial bulk of vastus lateralis. Femoral nerve motor branches reach the muscle from its medial and deep aspect, a pattern relevant in regional anesthesia and iatrogenic injury during anterior thigh procedures.
