Arising from the anterolateral arch of the cricoid cartilage, the vertical part (pars recta) of the cricothyroid muscle ascends superiorly to insert on the inferior margin and inferior horn (cornu inferius) of the thyroid cartilage. From a lateral perspective of the male larynx, the pars recta sits superficial to the cricothyroid joint, with the thyroid cartilage positioned superior and anterior to the cricoid, and the upper tracheal rings continuing inferiorly. The cricothyroid membrane and adjacent connective tissue occupy the interval between the thyroid and cricoid cartilages. Clean geometry. Functionally, the cricothyroid is the primary tensor of the vocal folds: contraction tilts the thyroid cartilage anteriorly and inferiorly relative to the cricoid, lengthening the vocal ligament and raising pitch. This lateral view matters when teaching how the external branch of the superior laryngeal nerve courses to innervate the cricothyroid and why it is at risk during thyroidectomy or ligation of the superior thyroid artery, producing loss of high-pitch phonation and vocal fatigue. It also helps distinguish the cricothyroid muscle from the more posterior intrinsic abductors and adductors that act directly on the arytenoid cartilages.
