Description
Oriented laterally on the left side, the descending (second) part of the duodenum is shown as a vertical intestinal segment continuous superiorly with the duodenal bulb and inferiorly turning toward the inferior duodenal flexure. Medially, its wall lies in close relation to the head of the pancreas, where the common bile duct and main pancreatic duct typically converge toward the major duodenal papilla. Posteriorly, the duodenum approaches retroperitoneal structures, while anteriorly it is crossed by peritoneal reflections and adjacent small-bowel loops depending on the degree of fixation. Retroperitoneal by habit. This lateral orientation matters because the second part of the duodenum is the operative corridor for bile and pancreatic duct access. Endoscopic cannulation during ERCP targets the major papilla on its posteromedial aspect, and small changes in rotation or fixation can alter the approach and raise the risk of post-ERCP pancreatitis or perforation. Surgeons also use this segment as a landmark during a Kocher maneuver, mobilizing the duodenum and pancreatic head to expose the inferior vena cava, aorta, and right renal vessels in trauma or pancreaticoduodenectomy planning.
