Description
Arcing at the junction of the transverse colon and descending colon, the left colic flexure (splenic flexure, flexura coli sinistra) forms an acute bend in the left upper quadrant. From a lateral perspective, the transverse colon approaches from an anteromedial and slightly superior direction, then turns inferiorly into the descending colon along the left abdominal wall. Superiorly, the flexure sits immediately beneath the left hemidiaphragm, with close adjacency to the splenic region and the left colic (phrenicocolic) ligament supporting its inferior aspect. Radiologists and surgeons care about this bend because it is a fixed point where colonic mobility changes abruptly, so looping and torque during colonoscopy often occur here, and sharp angulation can make intubation of the descending colon more difficult. Ischemic colitis also clusters around watershed zones; the splenic flexure lies near the Griffiths point where perfusion between the middle colic and left colic arterial territories may be tenuous in low-flow states. Left-sided colectomy planning benefits from a clear mental map of the flexure’s relationship to the diaphragm and spleen, since traction at this corner can translate to capsular injury and bleeding.
