Hepatic (right colic) flexure anatomy is centered on the junction where the ascending colon turns sharply to become the transverse colon, shifting from a superiorly directed, retroperitoneal course to an anteromedial, intraperitoneal trajectory across the upper abdomen. Superior to the flexura coli dextra lies the inferior surface of the liver, and the gallbladder fossa commonly sits anteromedial to the bend. Posterior relationships typically include the right kidney and second part of the duodenum, with the hepatic flexure tethered by peritoneal reflections and the right transverse mesocolon, and supported inferiorly by the right colic (paracolic) gutter. For surgeons and endoscopists, this segment is a predictable point of angulation and fixation that can resist both colonoscope advancement and colonic mobilization. The hepatic flexure becomes a focal landmark during right hemicolectomy and extended right colectomy, where division of peritoneal attachments and careful dissection near the duodenum and Gerota fascia limit iatrogenic injury. It also matters in cross sectional imaging: subtle wall thickening or pericolic fat stranding here raises different differentials than similar findings in the sigmoid, and colonic interposition beneath the right hemidiaphragm (Chilaiditi sign) can complicate interpretation and procedures.
