Description
Arcing in the left upper quadrant, the colic flexure (flexura coli sinistra, splenic flexure) forms the sharp bend between the transverse colon and the descending colon. The transverse colon approaches from the right and runs anterior to the pancreas before turning inferiorly at the left colic angle, where the descending colon continues caudally along the lateral abdominal wall. Superior and lateral to the flexure lies the spleen, with the left kidney and diaphragm positioned posteriorly and the stomach situated anteromedially depending on distension. Peritoneal reflections at this corner, including the phrenicocolic ligament and the transverse mesocolon, frame the attachment points that limit mobility. Clinically, the splenic flexure is a watershed region between the superior mesenteric and inferior mesenteric arterial territories, making it a classic site for ischemic colitis after hypotension or low-flow states. It is also the most fixed and often the highest point of the colon, a frequent location for gas trapping and pain in colonic distension and a known challenge during colonoscopy when negotiating the acute angulation. For oncologic and inflammatory disease workups, its proximity to the spleen matters, splenic capsular injury is an established risk during difficult colonoscopy, left colectomy, or mobilization of the splenic flexure.
