Description

Prominent in the left upper quadrant, the splenic flexure (flexura coli sinistra) marks the sharp bend where the transverse colon turns inferiorly to become the descending colon. Its apex lies superior and posterior relative to the left colic (descending) segment, tucked beneath the inferior pole of the spleen and commonly draped by the greater omentum. Medially, the mesocolon approaches the pancreatic tail, while laterally the colonic wall abuts the diaphragmatic surface and parietal peritoneum near the left paracolic gutter. A high, fixed corner. The splenic flexure sits at a watershed between the middle colic and left colic arterial territories, so hypoperfusion can present with segmental ischemic colitis and pain in the left upper abdomen or along the left flank.
