Description
Prominent in the left upper quadrant, the splenic flexure (flexura coli sinistra) marks the sharp bend where the transverse colon turns inferiorly to become the descending colon. Its apex lies superior and posterior relative to the left colic (descending) segment, tucked beneath the inferior pole of the spleen and commonly draped by the greater omentum. Medially, the mesocolon approaches the pancreatic tail, while laterally the colonic wall abuts the diaphragmatic surface and parietal peritoneum near the left paracolic gutter. A high, fixed corner. That anatomic position explains its behavior in practice. The splenic flexure sits at a watershed between the middle colic and left colic arterial territories, so hypoperfusion can present with segmental ischemic colitis and pain in the left upper abdomen or along the left flank. Its superior, posterior relationship to the spleen also makes it a relevant landmark during left hemicolectomy and splenectomy, where traction or dissection can injure the splenic capsule, and during colonoscopy, where a tight angle can limit advancement and increase loop formation. Colorectal surgery texts and GI endoscopy training modules use this configuration to teach flexure mobilization, recognition of the left upper quadrant peritoneal reflections, and safe scope negotiation across the colic bend. Anatomy and radiology courses also reference the splenic flexure when correlating abdominal pain patterns and CT localization of colitis, diverticular disease extension, or obstructing left-sided tumors.
