Description
Curving beneath the inferior surface of the right lobe of the liver, the hepatic (right colic) flexure marks the superior transition where the ascending colon turns anteromedially to become the transverse colon. The ascending colon lies along the right lateral abdominal wall, while the transverse colon courses horizontally across the upper abdomen, suspended by the transverse mesocolon. Just superior and anterior sits the gallbladder and hepatic margin, with the second part of the duodenum and the right kidney positioned more posteriorly; omentum often drapes anterior to the colonic bend. This segment matters because the hepatic flexure is one of the more adherent portions of large bowel, commonly tethered by peritoneal reflections to the underside of the liver and to the retroperitoneum, so mobilization during right hemicolectomy starts with an intentional takedown along the white line of Toldt and progresses toward this upper-quadrant turn. Endoscopists also recognize the right colic flexure as a frequent point of looping and patient discomfort, where careful torque and patient repositioning help the scope traverse into the transverse colon. In obstruction, carcinoma at the hepatic flexure can present with proximal colonic dilatation and, when the ileocecal valve is competent, a closed-loop configuration that raises perforation risk at the cecum.
