Description
Curving at the left upper quadrant, the flexura coli sinistra (left colic flexure) marks the junction of the distal transverse colon with the proximal descending colon. The transverse colon approaches from the patient’s right to left, then turns inferiorly to continue as descending colon along the left lateral abdominal wall. Relative to the stomach and spleen, this bend lies inferior to the splenic hilum and posterolateral to the greater curvature region, with the adjacent mesocolon tethering the segment to the posterior abdominal wall. A fixed point. Surgeons and endoscopists care about this corner because the left colic flexure is a common technical bottleneck during colonoscopy, where angulation and partial fixation increase loop formation and patient discomfort compared with the more mobile transverse segment. Its vascular anatomy also carries real consequence: the region often coincides with a watershed between the middle colic territory (superior mesenteric artery) and the left colic territory (inferior mesenteric artery), so low flow states can manifest as ischemic colitis near Griffiths’ point. During left hemicolectomy or mobilization of the splenic flexure, understanding its relationship to the spleen and tail of pancreas helps prevent capsular splenic injury and pancreatic traction.
