Description
Vomer anatomy dominates this lateral skull-base close-up, with the vomerine groove (sulcus vomeris) cutting a shallow, obliquely slanted furrow along the side of the vomer’s thin plate. Superiorly, the vomer thickens toward its alae where it abuts the rostrum of the sphenoid, while the anterior border lies toward the junction with the perpendicular plate of the ethmoid and septal cartilage. Inferiorly, the vomer meets the nasal crests of the maxilla and palatine bones to form the posterior part of the hard palate, placing the groove on the lateral surface facing the nasal cavity rather than the midline septal plane. Septal surgery lives and dies by landmarks, and the vomerine groove provides one of the few consistent surface cues on an otherwise smooth, delicate bone. When correcting a deviated nasal septum, surgeons often elevate mucoperichondrium and mucoperiosteum off the vomer and adjacent septal cartilage, and the groove helps orient the plane of dissection along the bony septum to reduce mucosal tearing and postoperative septal perforation risk. Trauma patterns matter too, since posterior septal fractures can involve the vomer near its superior articulations, and lateral surface irregularities around the groove can explain persistent nasal obstruction after an apparently minor injury. Small structure, real consequences.
