Description
Beneath a translucent facial skin layer, the paired zygomaticus major muscles run obliquely from the zygomatic bone (malar eminence) inferomedially toward the lateral aspect of the upper lip and oral commissure. Superficial facial landmarks align with deeper bony references, including the orbits and nasal aperture superiorly and the maxilla and mandible framing the dentition anteriorly. Inferior to the mandible, the hyoid bone sits in the anterior neck, with upper cervical vertebrae visible posteriorly as a midline skeletal column. Understanding the zygomaticus major in an anterior facial view matters because its vector of pull determines commissure elevation during smiling and sets the balance between midface soft tissues and perioral expression. Injury to the buccal and zygomatic branches of the facial nerve (CN VII), iatrogenic during midface surgery or parotid procedures, can weaken or asymmetrically paralyze this muscle, producing a characteristic droop of the oral angle. The muscle’s relationship to the zygomatic bone and the modiolus also underpins aesthetic planning for facelift (rhytidectomy), midface lift, and botulinum toxin placement in the perioral region.
