Description
Beginning in an anterolateral neck view, the sternocleidomastoid is traced from the manubrium and medial clavicle superiorly to its insertion on the mastoid process and lateral superior nuchal line, draping obliquely over the anterolateral cervical spine. Deep and slightly posterior to it, the anterior, middle, and posterior scalene muscles rise from the transverse processes of C2 to C7 and descend to the first rib (anterior and middle) and second rib (posterior), creating a stepped muscular wall lateral to the vertebral bodies. As the animation progresses, the striated fiber direction and layered depth cue the relationship of these muscles to the articulated cervical vertebrae, clarifying what sits superficial, what sits deep, and what lies between. Clinically, this is the corridor where neck movement and neurovascular access intersect. Hypertrophy or spasm of the scalenes can narrow the interscalene triangle between the anterior and middle scalene, a common anatomic substrate for thoracic outlet syndrome affecting the brachial plexus and subclavian artery, while the subclavian vein courses anterior to the anterior scalene and behaves differently under compression. Motion matters here: seeing the cervical segments articulated while the sternocleidomastoid and scalenes maintain their attachments helps explain why rotation and lateral flexion can reproduce radicular symptoms, and why scalene blocks or botulinum toxin targets are placed with attention to depth and neighboring structures. A tight SCM changes the whole contour. Expect to use this sequence in cervical spine and neck modules for gross anatomy, kinesiology, and regional anesthesia teaching, and as supporting media for texts covering thoracic outlet syndrome, accessory nerve-related torticollis patterns, and ultrasound-guided interscalene approaches.
