Description
Progressive malignant change is traced through the male larynx, with the thyroid cartilage forming the anterior shield and the cricoid cartilage completing the inferior ring, while the arytenoid cartilages sit posteriosuperiorly on the cricoid lamina and anchor the true vocal folds. As the sequence advances, a laryngeal carcinoma expands as an irregular mass along the glottic region, distorting the vestibular (false) folds superiorly and narrowing the rima glottidis between the vocal folds. Cord symmetry breaks down as one vocal fold becomes thickened and tethered, with reduced medial excursion relative to the contralateral side during simulated abduction and adduction. Subtle deformation of the cartilaginous framework accompanies the mucosal lesion. A critical landmark. Hoarseness is often the first symptom of glottic cancer because even small lesions on the true vocal fold disrupt the mucosal wave and produce early vocal fold fixation, and the animation makes that mechanical relationship easier to grasp than a still laryngoscopic frame. As the tumor enlarges, the narrowed airway explains exertional dyspnea and inspiratory stridor, and the viewer can correlate changing cord position with likely involvement of the cricoarytenoid joint or the recurrent laryngeal nerve.
