Layered adipose tissue within the superficial fascia is animated as it builds the external contour of the female breast over the anterior thoracic wall. Superficial (subcutaneous) fat lies immediately deep to the skin and superficial to the mammary gland, while deeper fat compartments interdigitate with glandular lobules and converge toward the nipple and areola at the anterior surface. As the sequence progresses, fibrous septa (Cooper ligaments) are seen spanning from dermis to the deep fascial plane, partitioning fat and tethering the breast mound as it rests anterior to the pectoral fascia and the retromammary space. Clinical interpretation of breast shape starts with these planes. Cooper ligament orientation and the distribution of superficial versus deep fatty tissue explain common physical findings such as ptosis, skin tethering, and the dimpling seen when carcinoma infiltrates the suspensory septa and shortens them.
