Description
Centered in the middle mediastinum, the male heart is shown with its myocardium enclosed by the fibrous and serous pericardium and resting on the superior surface of the diaphragm. An anterior thoracic perspective establishes the sternum and costal cartilages anteriorly, the lungs and pleural reflections laterally, and the pericardiophrenic attachment inferiorly as the pericardial sac blends with the diaphragm’s central tendon. As the sequence progresses, subtle diaphragmatic excursion lifts and lowers the pericardial base, clarifying how the cardiac apex remains directed anteroinferiorly and to the left while the inferior (diaphragmatic) surface maintains broad contact with the diaphragm. That relationship matters at the bedside and in the OR. Pericarditis, pericardial effusion, and post-infarct Dressler syndrome often present with pain referred via the phrenic nerve (C3 to C5), and seeing the pericardium anchored to the diaphragm helps explain why deep inspiration and supine positioning can change symptoms. The animated movement also makes the subxiphoid corridor more intuitive by showing how the heart’s inferior surface and pericardium approach the diaphragm during expiration, a practical setup for subxiphoid pericardiocentesis and for understanding why diaphragmatic paralysis can alter cardiac silhouette and mediastinal shift on imaging.
