The apex is formed by the left ventricle and comes to rest deep to the left fifth intercostal space near the midclavicular line, with the base positioned more posterior and superior, related to the left atrium and the proximal great vessels. Pericardium and adjacent thoracic landmarks remain in register as the orientation settles, including the sternum anteriorly, the diaphragm inferiorly, and the medial margins of both lungs laterally. Orientation becomes obvious. That leftward tilt is not an artistic convention, it explains where you expect to find the point of maximal impulse and why palpation is easiest when the apex contacts the chest wall between costal cartilages rather than under the sternum. Shifts in the apex position are a daily bedside problem: left ventricular dilation can displace the PMI laterally and inferiorly, while hyperinflated lungs in COPD may blunt the impulse by interposing aerated lung between pericardium and ribs.
