Description
Centered in the pelvis, the uterus appears in an anatomic relationship with the adnexa, with the fundus superior to the cervix and the uterine body continuous inferiorly with the vaginal canal region. Lateral to the uterine cornu, the fallopian tube courses toward an ovary enlarged by a malignant neoplastic mass, distorting the normal almond shape and displacing adjacent peritoneal reflections. As the sequence progresses, the animation clarifies how the tumor expands within the ovarian envelope, abuts the mesovarium and broad ligament, and can crowd the uterine tube, utero-ovarian ligament, and infundibulopelvic (suspensory) ligament where the ovarian vessels travel. Spatial relationships are clear. Adnexal tumors are approached first as an anatomic problem: origin, mobility, and contact with the uterus, tube, and pelvic sidewall guide differential diagnosis and operative planning. Seeing the growth in motion helps you appreciate why ovarian carcinoma, including mucinous and clear-cell subtypes as well as malignant germ cell tumors, can present as a complex mass with mass effect rather than early mucosal symptoms, and how proximity to the tube and uterine serosa influences routes of local extension and peritoneal seeding.
