Description
Seen from an inferior perspective, the superficial and deep transverse perineal muscles span the posterior margin of the urogenital triangle, running from the ischial ramus and ischial tuberosity region medially toward the perineal body (central perineal tendon). The bulbospongiosus lies more medial and anterior around the bulb of the penis, while the ischiocavernosus hugs the ischiopubic ramus laterally along the crus. Posteriorly, the external anal sphincter encircles the anal canal, and the levator ani and coccygeus form a deeper muscular diaphragm that slopes superomedially toward the midline. Distal bony landmarks include the pubic arch anteriorly and the coccyx posteriorly. Orientation of these transverse perineal fibers matters because they define the stabilizing “tie-bar” of the perineal body and the posterior edge of the perineal membrane, a junction that surgeons and pelvic floor therapists return to repeatedly. Obstetric perineal laceration classification is taught in females, but the same perineal body anatomy in males anchors the external anal sphincter and bulbospongiosus, so disruption from perineal trauma, straddle injury, or iatrogenic dissection can contribute to pelvic floor pain and defecatory or ejaculatory dysfunction. For urologic procedures, this inferior view clarifies the relationship of the deep perineal pouch to the membranous urethra and the location where perineal urethrostomy or bulbar urethroplasty dissection approaches the midline raphe. Ideal for teaching pelvic floor anatomy in gross anatomy lab, urology and colorectal surgery modules, and for illustrating the perineal body in manuscripts on pelvic pain, continence mechanisms, or perineal surgical approaches.
